We understand that insurance coverage and out-of-pocket costs are concerns for many patients and can be difficult to understand. Our out-of-network and insurance policy can help you understand what services are covered by your insurance and help prevent unexpected charges.
We are committed to delivering top quality medical care for all metabolic and endocrine conditions. The following information applies strictly to office visit charges. This does not include bills you may receive from imaging facilities or procedures performed outside our office. Blood and urine samples collected in our office are generally sent to Northwell Labs for processing. Northwell Labs accepts most insurance plans, excluding out-of-state Medicare and Medicaid plans. Please inform our staff if you would like laboratory testing to be performed elsewhere.

Are you insurance-friendly?
Dr. Sarah Fishman is an out-of-network provider with all insurance providers except for government issued (original) Medicare*. If you do not have original Medicare as your primary insurance, we can help you to submit your bill to your insurance carrier**, but our services are always processed as out-of-network. In other words, we do not accept insurance, but we can help you to have some of your costs covered by insurance if your plan provides out of network coverage. No matter your coverage, we encourage you to consider your health first. With certain plans, you may be eligible for some coverage, however, we cannot guarantee approval from your insurance carrier.
For patients that do have Medicare insurance, there may be services or procedures that aren’t covered by your insurance. We will notify you before you receive uncovered services so that you can have the opportunity to evaluate if that service is right for you. You will receive a written estimate of the cost of the service, and you always opt to decline the service without any loss of your other medical care.
Please be advised, we do not currently participate with managed Medicare, Medicaid, Tricare, and Medicaid-sponsored plans.
Why do you choose to remain an out-of-network provider?
While insurance companies lower out-of-pocket costs, in-network providers are often faced with limited decision-making capabilities, ultimately affecting the quality of patient care. In order to avoid such limitations, Dr. Fishman chooses to remain as an out-of-network provider. As such, we are able to maintain our autonomy and ensure our patients receive optimal care catered to their individual cases.
How much does treatment cost?
Our fees for treatment vary depending on the services you receive and the severity of your condition. Fees for treatment can be estimated when you schedule an appointment.
What if I do not have out-of-network coverage?
With certain insurance plans, you may be eligible for coverage, however, we cannot guarantee approval by your provider. It is always best to review your coverage with your insurance provider. But keep in mind, if you do not have any insurance coverage, do not have out-of-network benefits, or cannot afford to pay for treatment, there are ways we may be able to help or obtain financing to cover the costs.
